Job summary

Open Minds Kempsey Medicare Mental Health Centre is seeking an Intake & Assessment Officer to welcome clients and guide them to the services they need. You will conduct intake, screening, and risk assessments, provide brief interventions and coordinate care with a multidisciplinary team.

Qualifications

• Qualifications in health, mental health, community services or related discipline.
• Experience conducting intake, screening, biopsychosocial and risk assessments.

Responsibilities

• Screen intake and assess risk for service users.
• Provide brief interventions and care coordination, with referrals as needed.
• Develop collaborative care plans and support access to services.
• Collaborate with local health, community and partner organisations.
• Participate in case reviews and supervision.
• Contribute to centre growth and community engagement.
• Work a fortnightly weekend roster.
