Job summary

Just Better Care Cairns is seeking an experienced and compassionate Care Partner to work closely with participants, families, support networks, and healthcare professionals to develop, coordinate, and review personalised care plans that promote independence, choice, and quality of life. As a Care Partner, you will ensure participant-centred, high-quality care and support services.

Qualifications

• Extensive experience in care coordination, case management or similar roles.
• Strong knowledge of aged care, disability and community services.
• Excellent communication and stakeholder management skills.
• Ability to work in multidisciplinary teams and manage documentation.
• Current licenses and checks as required (first aid, CPR, screening).

Responsibilities

• Conduct comprehensive participant assessments and care planning.
• Develop, implement, monitor and review individualised care plans.
• Coordinate services and supports to achieve participant goals.
• Maintain regular contact with participants, families and health professionals.
• Monitor wellbeing, risks, and participant budgets per requirements.
• Assist participants to access community resources and supports.
• Keep accurate records and documentation within required timeframes.
