Responsibilities

• Meet clients to understand their needs, concerns, and personal circumstances.
• Identify suitable community services and support options for clients and their families.
• Develop and coordinate support, education, and training programs to improve wellbeing and independence.
• Help clients access housing, health, employment, education, and other welfare services.
• Refer clients to government departments and other service providers for further assistance.
• Collaborate with community groups and agencies to address local issues.
• Encourage daily living skills and participation in community activities.
• Monitor progress and maintain accurate records of support and development.
• Prepare reports and documentation to support programs and services.
