Job summary

Claro Disability Services is seeking compassionate support workers to assist clients in Gippsland with complex care needs, mobility, daily tasks, and health monitoring.

You will collaborate with families and healthcare professionals to tailor goals and plans, delivering person-centered care with dignity and respect. Flexible shifts and travel between clients support meaningful, community-based work.

Responsibilities

• Person-centered planning: chat with the client (and family/advocates) to understand goals and routines.
• Complex physical care: assist with mobility, transfers, positioning, daily tasks, and devices per the plan.
• Medical supports: monitor changes, follow clinical plans, and liaise with the care team as needed.
• Social time and participation: provide meaningful activities aligned with client interests.
• Autonomy and advocacy: support decision-making and ensure consent.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

Who we’re looking for

• Supports you may be required to assist with: Diabetes management, Medication support, asthma management, manual handling, mental health and behaviours of concern, wound management, food prep and handling, Traumatic brain injury management, food prep and handling, Dysphagia, Mealtime management
• Full Australian Drivers Licence and a reliable vehicle (preferably small car i.e. sedan)

What shifts are we offering?

• Client 1 in Traralgon - Tuesday - fortnightly 9:30am-2:30pm
• Client in Moe - Tuesday - weekly 12pm-2:30pm / Wednesday - weekly 12:30pm-3:30pm / Thursday - weekly 12pm-3pm
• Client 2 in Traralgon - Tuesday - weekly 10am-2pm / Wednesday - weekly 10am-2pm / Thursday - weekly 10am-2pm / Saturday - weekly 10am-1pm
• Client in Churchill - Thursday - weekly 9am-12pm
