Claro Disability Services in Gippsland seeks a caring Disability Support Worker to help clients with complex needs across mobility, daily tasks and health support. You’ll tailor supports, collaborate with families and healthcare professionals, and adapt to changing shifts with a person-centred approach.

Qualifications

Responsibilities

• Chat with the client and family to understand goals and routines
• Assist with mobility, transfers, daily tasks and devices as outlined
• Monitor medical plans and liaise with care team for changes
• Facilitate social activities and community access aligned with interests
• Support autonomy and decision-making while respecting consent

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

Who we’re looking for

• Interest or experience in complex care, disability, and health nuances.
• Great communicator: listens well, reads cues, and adapts to different styles.
• Reliable and professional, with good notetaking and teamwork.

• Supports you may be required to assist with: Diabetes management, Medication support, asthma management, manual handling, mental health and behaviours of concern, wound management, food prep and handling, Traumatic brain injury management, food prep and handling, Dysphagia, Mealtime management
• Full Australian Drivers Licence and a reliable vehicle (preferably small car i.e. sedan)

What shifts are we offering?

• Client in Traralgon - Tuesday - fortnightly 9:30am-2:30pm
• Client in Moe - Tuesday - weekly 12pm-2:30pm / Wednesday - weekly 12:30pm-3:30pm / Thursday - weekly 12pm-3pm
• Client in Churchill - Thursday - weekly 9am-12pm
