Claro Disability Services in Cranbourne South is seeking dedicated carers to support clients with complex needs. You’ll provide person-centred care, assist with mobility, daily tasks, and medical supports while building meaningful, respectful relationships.

Responsibilities

• Communicate with clients and families to understand goals and routines.
• Assist with mobility, transfers, daily tasks, and use of equipment as per plan.
• Monitor health changes, follow care plans, and liaise with the care team.
• Facilitate social participation and community access.
• Support autonomy and advocate for client decisions and preferences.
• Respect cultural backgrounds and beliefs in all interactions.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

• Saturday 7:00pm to 11PM (S/O - 7AM) +7am to 10:00am next morning
• Sunday 7:00pm to 11PM (S/O - 7AM) +7am to 10:00am next morning

WK 2:

• Thursday 2:00pm to 7:00pm
• Saturday 2:00pm to 10:00pm

49 hours per fortnight
