Job summary

Claro Disability Services is seeking a Support Worker for the Clarence Valley community in New South Wales. The role involves person-centered planning, complex physical and medical supports, and facilitating meaningful social participation for clients with complex needs.

You will collaborate with families, medical professionals, and other support workers to tailor care to each client’s goals, routines, and culture, while maintaining flexibility across varying shifts and locations within the

Qualifications

• Person-centered, warm, patient and genuinely caring
• Interest or experience in complex care, disability, and health nuances
• Great communicator: listens well, reads cues, and adapts to different styles
• Reliable and professional, with good notetaking and teamwork
• Flexible and adaptable; comfortable with changing needs and shifts

Responsibilities

• Person-centered planning: Chat with the client (and family/advocates), understand goals and routines
• Complex physical care: Help with mobility, transfers, positioning, daily tasks and devices as outlined in the plan
• Medical supports: Monitor changes, follow medical/clinical plans, liaise with care team if needed
• Social time and participation: Provide meaningful community access aligned with interests
• Autonomy and advocacy: Support decision-making and encourage independence where possible
• Cultural humility: Respect backgrounds, beliefs, and ways of being
