Qualifications

• Person-centered, warm, patient and genuinely caring.
• Interest or experience in complex care, disability, and health nuances.

• Reliable and professional with good note-taking and teamwork.

Responsibilities

• Person-centered planning: chat with client/family to learn goals and routines.
• Complex physical care: mobility, transfers, positioning, daily tasks and devices.
• Medical supports: monitor changes, follow plans, liaise with care team.
• Social time and participation: provide meaningful activities aligned with interests.
• Autonomy and advocacy: support decision-making and independence where possible.
• Cultural humility: respect backgrounds and beliefs.
