Job summary

Surematch Pty is seeking an experienced Social Worker for an on-site role in Mount Isa, Queensland. You will provide practical and emotional support to individuals, families and the wider community, with a strong focus on independence, resilience and wellbeing.

Relocation support is available for the successful candidate, and you will work both independently and as part of a collaborative team in a rural setting, applying ethical practice and sound judgement.

Qualifications

• Qualified in social work with a recognised degree and eligibility for professional membership.
• Experience in case management, client assessment and care planning in community, health or social services.
• Ability to respond to changing client and community needs with a flexible approach.

Responsibilities

• Conduct client assessments to identify needs at individual, family and community level.
• Develop and implement appropriate support and care plans.
• Provide person-centred support across a diverse caseload.
• Deliver crisis intervention, advocacy and referrals to services.
• Connect clients with community services and government programs.
• Collaborate with multidisciplinary teams and local organisations.
• Participate in case reviews for continuity and quality of care.
• Maintain confidential, accurate case records and documentation.
