Job summary

Respect in Warrnambool is seeking a Coordinator Community Programs to lead the delivery of community programs and HACC-PYP services, helping eligible participants to stay active, build social connections and achieve their goals.

This is a Monday–Friday, office-based role in Warrnambool. You’ll report to the Manager Wellness & Reablement Services, coordinating participant services, working with families, staff and community partners to deliver outcomes and uphold quality and policy requirements.

Qualifications

• Experience in aged care or community services.
• Experience coordinating services, programs or participant supports.
• Strong person-centred care, wellness and reablement knowledge.
• Excellent organisation and time management.
• Strong communication with participants, families and stakeholders.
• Proficiency with client management systems and Microsoft Office.
• Current First Aid, police check and a valid driver’s licence.
• Certificate IV or higher in Community Services or related field.

Responsibilities

• Coordinate community programs and services that promote independence, wellbeing and meaningful participation.
• Manage referrals, onboarding, assessments, service commencement and ongoing reviews.
• Develop and maintain individual care plans and goals reflecting changing needs.
• Connect participants with programs and opportunities that match interests and needs.
• Build positive relationships with participants, families, carers, volunteers and partners.
• Coordinate program operations, schedules, resources and communications.
• Monitor service quality and participant outcomes, maintain records and escalate risks.
• Champion wellness and reablement, supporting continuous improvement and compliance.
