Job summary

Applicants should have a Certificate IV in Lifestyle and Health or be working towards it, with knowledge of aged care, accreditation standards, and programs like MAC/CHSP/HACC/NDIS.

Qualifications

• Certificate IV in Lifestyle and Health or working towards.
• Knowledge of wellness and reablement in aged care with goal setting and care planning.
• Familiarity with accreditation standards, quality systems and continuous improvement.
• Working knowledge of MAC, CHSP, HACC, SAH, NDIS and related programs.

Responsibilities

• Deliver activities programs in partnership with clients and group coordinator.
• Assist with transport of clients to the centre and activities.
• Deliver inclusive and welcoming activities for clients of varying abilities.
• Support the group and participate in on-site or community-based sessions.
