Job summary

Respect in Warrnambool is seeking a Coordinator Community Programs to lead the delivery of community programs and HACC-PYP services, helping eligible participants to stay active, build social connections and achieve their goals.

This is a Monday–Friday, office-based role in Warrnambool. You’ll report to the Manager Wellness & Reablement Services, coordinating participant services, working with families, staff and community partners to deliver outcomes and uphold quality and policy requirements.

Qualifications

• Experience in aged care or community services.
• Experience coordinating services, programs or participant supports.
• Strong person-centred care, wellness and reablement knowledge.
• Excellent organisation and time management.
• Strong communication with participants, families and stakeholders.
• Proficiency with client management systems and Microsoft Office.
• Current First Aid, police check and a valid driver’s licence.
• Certificate IV or higher in Community Services or related field.

• Manage referrals, onboarding, assessments, service commencement and ongoing reviews.
• Develop and maintain individual care plans and goals reflecting changing needs.
• Connect participants with programs and opportunities that match interests and needs.

• Coordinate program operations, schedules, resources and communications.
• Monitor service quality and participant outcomes, maintain records and escalate risks.

About the Role

This is a Monday–Friday, office-based role located in Warrnambool.

As our Coordinator Community Programs, you’ll lead the coordination and delivery of community programs and HACC-PYP services, that create meaningful opportunities for eligible participants to achieve their goals, build social connections and remain active in their communities.

Reporting to the Manager Wellness & Reablement Services, you’ll coordinate participant services, facilitate access to activities and programs, and work closely with families, employees, volunteers and community partners to achieve positive outcomes.

You’ll bring strong organisational and relationship-building skills to ensure services are responsive, effective and aligned with each participant’s needs. You’ll also support the continuous improvement and smooth operation of community-based programs.

Championing wellness and reablement, you’ll work collaboratively to deliver services that reflect our organisational values and meet relevant quality, legislative and policy requirements.

No two days are the same! As Coordinator Community Programs you will:

• Coordinate community programs and services that promote independence, wellbeing, social connection and meaningful participation.
• Manage the participant journey, including referrals, onboarding, assessments, service commencement and ongoing reviews.
• Develop and maintain individual care plans and goals, ensuring services reflect changing needs, preferences and desired outcomes.
• Connect participants with programs and community opportunities that match their interests, goals and support needs.
• Build positive relationships with participants, families, carers, volunteers and community partners, responding to enquiries and feedback with care and professionalism.
• Coordinate program operations, including schedules, resources, communications and service providers, to support smooth and effective delivery.
• Monitor service quality and participant outcomes, maintaining accurate records, identifying risks and escalating issues when required.
• Champion wellness and reablement, contributing to continuous improvement, quality initiatives, community engagement and compliance requirements.

About You

You’ll thrive with us if you have:

• Experience in aged care, community services, disability, health or a related sector.
• Experience coordinating services, programs, activities or participant supports.
• A strong understanding of person-centred care, wellness and reablement approaches.
• Strong relationship-building skills, with the ability to work positively with participants, families, carers and stakeholders.
• Excellent organisation and time management skills, with the ability to manage competing priorities.
• Confident written, verbal and interpersonal communication skills.
• The ability to work independently and collaboratively as part of a team.
• Strong attention to detail and experience maintaining accurate records, documentation and reports.
• Confidence using client management systems and Microsoft Office.
• Current First Aid certification, National Police Check (or willingness to obtain) and a current Driver Licence.
• A Certificate IV, Diploma or higher qualification in Community Services, Aged Care, Health, Leisure & Lifestyle or a related field.
• Experience coordinating community-based or social support programs.
• Knowledge of government-funded community care programs and related reporting requirements.
• Experience working with older people in a community, health or care setting.
