Job summary

Community Services #1 in Canberra is seeking a Community Services Team Leader to coordinate day-to-day delivery of programs, allocate participants and monitor workflows in line with funding and organisational requirements.

You will lead staff to ensure service delivery aligns with quality standards, assist with documentation, and support teams in interpreting program requirements for continuous improvement and strong community impact.

Qualifications

• Diploma qualification in Community Services, Disability Support, Aged Care, Social Work, Health Services or related field.
• At least two years’ relevant experience coordinating community or health services.
• Experience handling referrals, assessments, care planning and transitions.

Community Services Team Leader – Full Time Ongoing

Community Services #1 is seeking a Community Services Team Leader to contribute to meaningful care that enhances independence and wellbeing. The position exercises initiative and judgement within established relevant community services program requirements, funding and organisational requirements and assists with monitoring program performance, risk, quality, outcomes and reporting.

About the role

• Coordinate the day-to-day delivery of relevant community services programs, including receiving referrals, allocating participants, establishing operational priorities and monitoring workflow and service timeframes.
• Monitor service delivery and participant documentation to support compliance with relevant community services program requirements, funding agreements, service specifications, organisational policies, legislation and quality standards.
• Support employees to interpret and apply relevant community services program requirements, operational procedures, quality requirements and documentation standards.

About you

• Relevant Diploma qualification in Community Services, Disability Support, Aged Care, Social Work, Health Services or a related field, with at least two years’ relevant experience.
• Demonstrated experience coordinating community or health services, including referrals, assessment, care planning, service coordination and participant transitions.

This is a full-time, ongoing role working 76 hours per fortnight. Remuneration is aligned with the SCHADS Award Level 5, plus superannuation and salary packaging benefits.

Requirements
