Job summary

Community Services #1 in Canberra is seeking a Community Services Team Leader to coordinate day-to-day delivery of programs, allocate participants and monitor workflows in line with funding and organisational requirements.

You will lead staff to ensure service delivery aligns with quality standards, assist with documentation, and support teams in interpreting program requirements for continuous improvement and strong community impact.

Qualifications

• Diploma qualification in Community Services, Disability Support, Aged Care, Social Work, Health Services or related field.
• At least two years’ relevant experience coordinating community or health services.
• Experience handling referrals, assessments, care planning and transitions.

Responsibilities

• Coordinate day-to-day delivery of community services programs, including referrals and participant allocation.
• Monitor service delivery and participant documentation for compliance with requirements and quality standards.
• Support staff to interpret and apply program requirements and procedures.
