Uniting NSW & ACT is seeking a dedicated Case Manager to lead delivery of person-centred home and community care services across the ACT. You will coordinate care planning, support clients to stay in their homes longer, and be the primary contact for client needs, funding considerations, and team collaboration.

The role involves travel across ACT and surrounding areas, and requires strong communication with clients, families, and healthcare partners.

Qualifications

• Experience in aged care and Home and Community Care case management.
• Ability to plan services within funding budgets.
• Experience communicating with clients and families.

Responsibilities

• Lead case management and delivery of person-centred home and community care services.
• Coordinate care planning to support clients to live at home.
• Act as central point of contact for clients and coordinate service delivery.
