Job summary

ACH Group is seeking a passionate Community Connector to lead our Social Experiences team and support older people to build social connections and engage with their communities.

This full-time role is based at Elizabeth on Mondays and across ACH Group locations for the rest of the week, involving travel between residential care homes to backfill as needed. You will develop social plans and run engaging activities to enhance wellbeing and reduce isolation.

  • Capacity to work flexibly in partnership with customers and others using a strong customer service approach
  • Experience coordinating small to medium sized events
  • Experience with people at risk of social isolation or memory loss/dementia

About the Role

We're looking for a passionate and experienced Community Connector to lead our Social Experiences team while supporting older people to build meaningful social connections and engage with their communities.

This is a full-time position based at Elizabeth on Mondays and based across multiple ACH Group locations for the remainder of the week.

In this rewarding role, you'll support/backfill a dedicated team, develop individual social plans, facilitate engaging group activities, and help create inclusive social opportunities that enhance wellbeing and reduce social isolation.

What you’ll do
  • Provide support the social team to deliver social experiences
  • Create social plans and reviews that identify social goals
  • Support the effective delivery of the social program across ACH Group residential sites, to facilitate Good Lives for our customers living in residential care.
  • Lead and run groups
  • Assist in organising and running of events
  • Orientate and supervise volunteers
About You!
  • You are talented and passionate about connecting people socially. You want to make the most of your expertise, do what you do best and thrive on seeing people have fun, engage in interests and connect with community.
  • Experience in leading and facilitating groups and/or events and supporting participation of group members
  • Capacity to work flexibly in partnership with customers and others using a strong customer service approach
  • Computer skills to complete documentation to meet accreditation standards.
  • Experience in the coordination of small to medium sized events
  • Experience working with people at risk of social isolation and loneliness including those from a CALD background and people living with memory loss or Dementia
  • Willingness to travel between residential care homes to backfill as part of the social team
You will need
  • Leadership and excellent team working experience
  • Hold or working towards a Certificate IV Community Services / Aged Care or equivalent relevant qualification
  • A satisfactory National Criminal History Record Check that complies with ACH Group Policy no older than 12 months at commencement of employment.
