Overview

Social Worker - Health & Community Services at ACH Group. The role involves providing essential social work services in the community, working directly with customers and their families to support social and health needs.

Available as full or part-time, you’ll work as part of a multi-disciplinary team as the sole Social Worker to support and empower customers in navigating social and health challenges, enabling them to lead healthy, independent lives while maintaining connections to their community. You will work across metropolitan regions to ensure services reach customers in diverse settings.

Key Responsibilities
  • Provide social work services within a community setting, empowering customers to make informed decisions about their care.
  • Conduct assessments, monitor progress, and review care and support plans to ensure customers’ needs are met.
  • Apply a wellness and reablement approach to service delivery, supporting customers’ overall wellbeing.
  • Collaborate with a multi-disciplinary team to achieve optimal health outcomes for customers.
  • Work closely with internal and external stakeholders to design services for customers’ individual needs.
  • Maintain accurate documentation and record-keeping in compliance with best practices and regulatory standards.
  • Other duties as required.
About You / Qualifications
  • Bachelor’s degree in social work and eligibility for membership with the Australian Association of Social Workers (AASW).
  • Minimum of 3 years’ experience as a Social Worker.
  • Proven ability to empower customers to make decisions and advise on available services.
  • Strong problem-solving skills with the ability to assess, set goals, and evaluate outcomes.
  • Excellent relationship-building skills and ability to work collaboratively.
  • Knowledge of aged care and older person’s health and needs.
  • Strong documentation skills with a high degree of accuracy.
  • A satisfactory police clearance for working in aged care (less than 12 months old) is required before commencement.
  • Not suitable for new graduates; relevant experience in community or health settings is required.
