Job summary

ACH Group in Mile End, South Australia, is seeking a full-time Care Coordinator to support older people in the community and enhance their quality of life.

You will work closely with a multidisciplinary team to tailor Home Care Packages, perform assessments, develop and review support plans, implement care interventions, and liaise with carers, health professionals and community services to promote independence and wellbeing.

Qualifications

• Diploma in Community Services Coordination or Nursing/Allied Health qualification
• Current registration with AHPRA if Nursing/Allied Health professional
• Driver's licence and a current police check for Aged Care (≤12 months)

Responsibilities

• Support customers with understanding, developing and reviewing Home Care Packages
• Conduct assessments, develop support plans, undertake reviews, implement and facilitate care interventions
• Promote ACH Group's services and solutions
• Liaise with external stakeholders including carers, medical and allied health professionals

ACH Group has a fulltime Care Coordinator role available to provide support and enhance the quality of life of our customers based at Mile End.

The role:

ACH Group has a full-time opportunity for a Care Coordinator to support older people in the community and enhance their quality of life. Working closely within a multidisciplinary team you will collaborate to provide tailored service options that promote independence, choice, wellbeing, and overall quality of life for older individuals living in the community.

What you'll do:

• Support customers with understanding, developing and reviewing their Home Care Package
• Conduct assessments, develop support plans, undertake reviews, implement and facilitate clinical and care interventions
• Promoting ACH Groups Services and Solutions
• Liaise effectively with external stakeholders, including carers, acute, respite and other community services, medical and allied health professionals
• Provide robust budget management, ensuring that accurate budgets are maintained so the customer can be supported as required

• Ability to assist individual requirements, including customers with a range of complex needs

• Excellent interpersonal, communication, negotiation, and problem-solving skills
• Competent using the MS Office Suite and learning to use a range of electronic tools and databases

You will need

• A Diploma in Community Services Coordination, Diploma / Certificate IV in Community Services, or similar equivalent qualification
• OR a Nursing or Allied Health qualification and current registration with AHPRA qualified Enrolled / Registered Nurse or Allied Health Professional
• Drivers licence
• A current police check for Aged Care, less than 12 months old
