Job summary

Encircle Community Services in Lawnton, Queensland, is seeking a dedicated Family Support Practitioner for a full-time, 3-month SCHADS contract. The role provides strengths-based, trauma-informed support to families across the Moreton Bay region, with office-based and outreach duties.

You will work within our Families Team, developing goals, linking clients to supports, and advocating for families while building community partnerships.

Qualifications

• Qualification in Social Work, Human Services, Psychology, Social Science or related discipline (Diploma or higher).
• Experience with children, young people and families in a community services setting is desirable.
• Knowledge of strengths-based, trauma-informed and family-focused practice.

Responsibilities

• Provide family support and case coordination using strengths-based, child-centred approaches.
• Develop and review support plans and goals with families.
• Undertake assessments and identify appropriate supports and referral pathways.
• Maintain accurate case notes, records and reporting requirements.
• Advocate for families and help navigate community services.
• Facilitate and support group programs and community activities.
• Build relationships with local stakeholders and partner organisations.
