BlueCare is seeking a Care Partner to coordinate person-centred care and services for clients in the community, spanning New Farm, Hamilton, Kedron, and surrounding areas. You will assess needs, develop and review care plans, coordinate services and transitions, manage budgets, and monitor risks on the road, while supporting clients to navigate the care system.

• Coordinating services and care transitions with clients, families and multidisciplinary teams.
• Managing client care budgets and available funding.
• Monitoring changing needs and risks, escalating to clinical teams where required.
• Reviewing care plans, services and outcomes.
• Supporting clients to navigate the care system and access appropriate services.
