UnitingCare Children, Young People and Families (BlueCare) is seeking a Care Partner to coordinate person-centred care for clients using aged care funding. You will assess needs, develop individualised care plans and work with families and multidisciplinary teams to ensure best outcomes.

You will travel in the Gympie region to support clients in their homes and communities, manage care budgets and funding, and help clients navigate the care system with a compassionate approach.

Qualifikationen

• Certificate IV in a Health or Community Service discipline, essential.
• Current Australian Driver’s Licence, essential.
• Vehicle for work that is roadworthy and insured, essential.
• Knowledge of client-centred community care funding streams.
• Ability to collaborate with clinical and care teams.
• Aged care legislation and quality requirements knowledge.
• Strong communication, interpersonal and problem-solving skills.
• Ability to satisfy probity checks.

Aufgaben

• Assess client needs and develop individualised care plans.
• Coordinate services and care transitions with clients, families and multidisciplinary teams.
• Manage client care budgets and funding.
• Monitor changing needs and escalate to clinical teams.
• Review care plans, services and outcomes.
• Support clients to navigate the care system and access services.
• Spend majority of time in the community engaging with clients.
