UnitingCare BlueCare Community Care is seeking a Care Partner in Queensland to oversee care planning and coordinate services for clients in Bowen, Proserpine and surrounding areas. You will travel between offices and deliver person-centred, high-quality care, working with multidisciplinary teams to optimise outcomes and navigate funding opportunities.

The role requires a Certificate IV in health or community services, a valid Australian driver’s licence, a roadworthy vehicle, and strong

Qualifikationen

• Certificate IV in Health or Community Service—essential.
• Current Australian Driver’s Licence—essential.
• Vehicle to use for work—roadworthy and insured (Third Party Property minimum).
• Experience delivering client-centred, community-based care services.
• Ability to liaise with clinical and care teams to coordinate care and exercise professional judgment.
• Experience with community funding streams and programs.
• Excellent communication and clinical problem-solving skills.
• Ability to manage time effectively and work autonomously.
• Ability to satisfy probity checks as required by policy.

Aufgaben

• Develop and review care plans in collaboration with clients and the care team.
• Manage budgets and facilitate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs of complex clients.
• Continuously monitor, review and evaluate care plans, services and outcomes.
• Assist clients in navigating the care system and linking to resources as needed.
