Job summary

BlueCare, part of the UnitingCare family in Australia, is seeking a Care Partner to oversee care planning and coordination for community clients across Bowen, Proserpine and surrounding areas. The role involves extensive driving between locations and building strong relationships with clients and clinical teams.

You will develop care plans, manage budgets, monitor client needs, and connect clients with resources to optimise outcomes.

Qualifications

• Certificate IV in a Health or Community Service discipline (Essential).
• Demonstrated understanding of client-centred, community-based care services delivery.
• Excellent communication and clinical problem-solving skills.
• Confidence to work autonomously and manage time effectively.
• Willingness to satisfy probity checks as required by legislation or policies (Essential).

Responsibilities

• Develop and review care plans with clients and the care team.
• Manage budgets and facilitate multidisciplinary care activities.
• Liaise with clinical teams to respond to changing needs of complex community clients.
• Continuously monitor and evaluate care plans, services and outcomes.
• Assist clients in navigating the care system and connecting with resources.
