Job summary

BlueCare is seeking a Clinical Care Partner to coordinate complex clinical and care needs for clients across Arundel, Southport and surrounding areas. You will travel between sites, develop and review care plans, coordinate services, and monitor outcomes to achieve optimal care.

The role requires current AHPRA registration (RN or Allied Health), strong communication, time management, and the ability to work autonomously in the community. A driver’s license and vehicle are essential.

Qualifications

• Registered Nurse or Allied Health professional with AHPRA registration.
• Demonstrated client-centred community-based care delivery knowledge.
• Driver's license and ability to drive for work commitments.

Responsibilities

• Develop and review complex care plans with clients and the care team.
• Manage budgets and coordinate multidisciplinary activities.
• Liaise with clinical teams to monitor changing needs of clients.
• Help clients navigate the care system and access resources.
