BlueCare in Gympie region is seeking a Care Partner to coordinate person-centred care and services for clients funded by aged care. You’ll assess needs, develop care plans, coordinate services and budgets, and monitor risks.

You'll spend significant time on the road meeting clients in the community, collaborating with clinical teams, families and care staff to ensure outcomes. This permanent full-time role offers not-for-profit salary packaging and benefits.

Formación

• Essential Certificate IV in Health or Community Service discipline.
• Current Australian Driver's Licence.
• Vehicle for work with roadworthy and insured (Third Party Property minimum).
• Experience and knowledge of client-centred community care and funding streams.
• Ability to collaborate with clinical and care teams to coordinate care and exercise professional judgement.
• Knowledge of relevant aged care legislative, regulatory and quality requirements.
• Strong communication, interpersonal and problem-solving skills.
• Ability to satisfy the relevant probity checks required by legislation or policies.

Responsabilidades

• Coordinate person-centred care and services to support clients to achieve outcomes from aged care funding.
• Assess client needs and develop and review individualised care plans.
• Coordinate services and care transitions with clients, families and multidisciplinary teams.
• Manage client care budgets and available funding.
• Monitor changing needs and risks and escalate to clinical teams when required.
• Review care plans, services and outcomes.
• Support clients to navigate the care system and access appropriate services.
