Job summary

BlueCare, part of UnitingCare, is seeking a Clinical Care Partner to oversee and coordinate complex clinical needs for community-dwelling clients in Coomera, Pimpama and surrounding areas. The role focuses on assessment, care planning and linking clients with necessary services to achieve optimal outcomes.

You will spend most days out in the community, working with multidisciplinary teams, managing plans, budgets and care delivery, and supporting client navigation through the care system.

Qualifications

• Essential: Registered Nurse or Allied Health professional with current AHPRA registration and unlimited work entitlements
• Essential: Current Australian Driver’s Licence
• Essential: Roadworthy and insured vehicle for work use
• Demonstrated understanding of client-centred, community-based care services delivery
• Demonstrated knowledge of Community Funding streams and programs
• Excellent communication and clinical problem-solving skills
• Ability to work autonomously and manage time effectively

Responsibilities

• Develop and review complex care plans in collaboration with clients and multidisciplinary teams
• Manage budgets and facilitate multidisciplinary care activities
• Liaise with clinical teams to monitor changing needs of complex community clients
• Continuously monitor, review and evaluate care plans, services and outcomes
• Assist clients in navigating the care system and linking to resources
