Job summary

Care and Connects Pty Ltd is seeking a compassionate Clinical Registered Nurse with leadership skills to join our dedicated team in Adelaide. You will oversee clinical care, medication compliance and risk mitigation while guiding a team to deliver high-quality, participant-centered care.

Responsibilities include conducting health assessments, developing and reviewing care plans, delivering training, and collaborating with participants, families and staff to achieve exceptional health outcomes.

Qualifications

  • Bachelor of Nursing and AHPRA registration.
  • Minimum of 3 years’ experience in aged care or disability services.
  • Valid First Aid/CPR, WWC, NDIS Worker Screening, Police Check and Australian Driver’s License.

  • Provide leadership to support workers, ensuring best practices in clinical care.
  • Deliver health-related education and training to participants and caregivers.

About the Role:
In this dynamic position, you’ll provide comprehensive healthcare support to participants in line with NDIS guidelines. You’ll oversee clinical care, medication compliance, and risk mitigation while leading a team committed to delivering high-quality, participant-centered care. Key responsibilities include conducting health assessments, developing care plans, delivering training, and collaborating with internal and external stakeholders to achieve exceptional health outcomes.

Key Responsibilities:

  • Conduct detailed health assessments and develop individualised care plans.
  • Provide leadership to support workers, ensuring best practices in clinical care.
  • Deliver health-related education and training to participants and caregivers
  • Develop, implement, and regularly review individual care plans in collaboration with participants, their families, and Management.
