Job summary

Ageing Australia is seeking a skilled Allied Health professional to join our not-for-profit aged care team in Adelaide. You will partner with clients to develop personalised care plans, coordinate services, and manage transitions to support independence, wellbeing and quality of life.

Qualifications

• Tertiary qualification (Bachelor's degree or higher) in Allied Health or Nursing.
• Current and unrestricted registration with AHPRA or other relevant professional body.
• Minimum two years' post-registration clinical experience in aged care, community health, rehabilitation, hospital transition care, palliative care, mental health or a related setting.
• Current driver's licence
• National Police Check

Responsibilities

• Manage a caseload of clients with diverse and often complex needs.
• Coordinate services and support care transitions.
• Contribute to multidisciplinary care planning that promotes independence, wellbeing, and quality of life.
• Apply clinical expertise to influence practice, discharge planning and service delivery improvements.
• Provide clinical guidance and support to Care Partners for high-quality, person-centred care.
• Identify risks and barriers to outcomes and influence improvements in care planning and delivery.
