Job summary

UnitingSA in Australia is seeking a motivated Care Partner to join our Community Aged Care team, with a strong focus on onboarding new clients and growing services. You will guide clients through My Aged Care, co-design personalised care plans, coordinate services to promote independence, and monitor delivery to meet funding and quality standards.

The position is initially covering maternity leave with possibility to extend, offering meaningful work with a not-for-profit organisation that values

Qualifications

• Strong relationship-building and communication skills to engage new clients and referrers.
• Experience or enthusiasm in client acquisition and service growth within health or aged care.
• Commitment to helping older people live well at home.
• Understanding of My Aged Care and community aged care services.

Responsibilities

• Proactively engage with prospective clients and families to promote UnitingSA’s aged care services.
• Lead onboarding and sign-up of new clients, guiding them through My Aged Care and service entry.
• Coordinate services to promote independence and support client goals.
• Co-design and manage personalised care plans aligned with My Aged Care assessments.
• Collaborate with clients, carers, and internal teams to deliver goal-oriented care.
• Monitor service delivery to ensure funding, quality standards, and legislative compliance.
• Identify and escalate high-risk situations per UnitingSA policies.
