Job summary

Princes Court Ltd is seeking a qualified Care Partner to join our Support at Home team in delivering high-quality, person-centred home care. You will coordinate care plans, navigate the My Aged Care system with clients, and work in a supportive, team-based environment to promote independence for older people.

The role involves collaborating with interdisciplinary teams, providing pathway planning across Restorative and End-of-Life care, and liaising with participants and their supporters.

Qualifications

• Diploma-level qualification in Community Services, Aged Care, Case Management or equivalent experience.
• Enrolled Nurses with community experience are highly desirable.
• Current Class C Driver's Licence with a reliable vehicle.
• COVID vaccinations x 2 mandatory for PCL.
• A current National Police Check/NDIS Worker Screening or willingness to obtain one.
• Experience in care coordination or case management in aged/health/disability services.
• Understanding of the Support at Home Program, Quality Standards and rights.
• Experience with quality improvements in Aged/Community Care or health environments.
• Excellent IT, computer and database skills including rostering software.
• Strong communication and interpersonal skills including report writing.
• Good time management and problem-solving abilities; prioritising workload.
• Familiarity with My Aged Care portals and digital case management systems.

Responsibilities

• Operate a high quality, financially sustainable home care service.
• Support clients from initial meeting through My Aged Care navigation.
• Develop goals to promote choice, control and independence.
• Collaborate with participants, supporters and interdisciplinary teams.
• Plan and oversee care delivery across Restorative, End-of-Life, AT-HM streams.
• Work with Clinically qualified Care Partners for complex care and local visits.
• Coordinate tailored, responsive and inclusive support.
