Job summary

AnglicareSA in South Australia seeks an Assessment Partner - Community Aged Care to conduct in-home assessments and guide care planning within a multidisciplinary team.

You will coordinate visits, support independence and provide clinical guidance on complex care needs, contributing to person-centered care and better quality of life for seniors in their homes.

Qualifications

• Two years minimum experience working autonomously in the field.
• Ability to conduct thorough assessments with sound clinical reasoning.
• Support independence and re-enablement for older people at home.
• Experience in community aged care or complex care settings.

• Coordinating your own calendar and managing daily visits across the community

About the Candidate

We're looking for a confident and experienced clinician with a minimum of two years working autonomously in the field to take on this role that will work with clients in the community while staying connected to a supportive, multidisciplinary team.

You will enjoy getting results for your clients as a result of your ability to conduct thorough assessments and applying sound clinical reasoning skills, which in turn will help older people maintain independence and quality of life at home.

You're someone who:

• Builds trust quickly with customers, families and stakeholders
• Is confident working independently and making decisions in complex or changing situations
• Has proven experience managing a varied case load of clients along with the associated workload
• Communicates with clarity, empathy and professionalism,
• Tertiary qualification in Nursing, Allied Health or related field
• Current AHPRA registration (where applicable)
• Experience in community aged care or complex care settings
• Strong assessment, care planning and clinical decision-making skills
• Ability to work autonomously and manage competing priorities
