Job summary

Ageing Australia is seeking a skilled Allied Health professional to join our not-for-profit aged care team in Adelaide. You will partner with clients to develop personalised care plans, coordinate services, and manage transitions to support independence, wellbeing and quality of life.

Qualifications

• Tertiary qualification (Bachelor's degree or higher) in Allied Health or Nursing.
• Current and unrestricted registration with AHPRA or other relevant professional body.
• Minimum two years' post-registration clinical experience in aged care, community health, rehabilitation, hospital transition care, palliative care, mental health or a related setting.
• Current driver's licence
• National Police Check

JOB DESCRIPTION

Job Description

You'll partner with clients to understand what matters most and support them to achieve meaningful goals through strengths-based, person-directed care. Managing a caseload of clients with diverse and often complex needs, you'll coordinate services, support care transitions, and contribute to multidisciplinary care planning that promotes independence, wellbeing, and quality of life.

Beyond direct client support, you'll apply your clinical expertise to influence practice, contribute to complex case reviews and discharge planning, and identify opportunities to improve service delivery. Working collaboratively across ECH, you'll help shape innovative approaches that enhance outcomes and enable older people to live well at home.

You'll make an impact by

Partnering with clients to develop personalised care plans and meaningful goals that promote independence, wellbeing, and quality of life.

Leading care coordination and service transitions for clients with complex needs, ensuring safe, seamless support across health and community services.

Driving reablement outcomes by supporting recovery, building capability, and helping clients maintain or regain functional independence.

Providing clinical guidance and support to Care Partners, strengthening capability and ensuring high-quality, person-centred care delivery.

Identifying risks, addressing barriers to client outcomes, and influencing improvements in care planning, service delivery, and clinical practice

About the Candidate

Tertiary qualification (Bachelor's degree or higher) in Allied Health or Nursing

Current and unrestricted registration with AHPRA or other relevant professional body

Minimum two years' post-registration clinical experience in aged care, community health, rehabilitation, hospital transition care, palliative care, mental health or a related setting

Experience supporting reablement, restorative care, rehabilitation, care management or complex client outcomes in community, health or aged care settings.

Current driver's licence

National Police Check
