Job summary

Help At Home in Sydney is seeking a Clinical Care Partner to support vulnerable and elderly clients across the Eastern Suburbs. You will draw on your nursing or allied health background to deliver community-based care within a not-for-profit framework.

Responsibilities

• Proven experience with complex clients, needs assessments and risk/safety assessments.
• Create and maintain the client's case management plan with assessments and service needs.
• Advocate on behalf of clients to organisations to ensure services meet government funding.
• Develop, implement, monitor and review case plans with clients and families.
• Provide comprehensive assessments to identify client needs using internal and external networks.
• Follow up on leads generated through word of mouth and marketing initiatives.

Clinical Care Partner for Support at Home provider

Are you an Allied Health or Registered Nurse professional who is ready to change career paths and join a not-for-profit organisation which specialises in the delivery of Home Care Support across the Eastern Suburbs area Sydney. Draw on your previous experience working with patients within a clinical setting and adapt those skills to our community clients.

Responsibilities include:

Proven experience working with complex clients, needs assessments and risk/safety assessments

Create and maintain the client's case management plan including conducting assessments, and determining the services needed in collaboration with the client.

Advocate on behalf of our client to a variety of organisation and ensure that all services are deliverable against their government funding

Develop, implement, monitor and review case plans in collaboration with theclient and families

Provide comprehensive assessments to identify client needs and requirements for clients using internal and external service networks.

Following up on leads that are generated through word of mouth, marketing and other initiatives.

To be successful you must:

Have experience and an understanding of Support at Home Program

Have a tertiary qualification in either Nursing, Allied Health, Disability or Gerontology

Be organised and have strong attention to detail
