Job summary

First Call Nursing is seeking a Case Manager to deliver person-centred case management for aged care and NDIS clients. The role involves coordinating services with clients, families, and providers, maintaining records, and ensuring compliance with funding and policy requirements.

The successful candidate will work independently within a small, busy team. You will manage an allocated client caseload, monitor outcomes and collaborate with various health professionals to deliver timely supports

Qualifications

• Demonstrated experience in case management or a similar role.
• Experience working within Aged Care and NDIS environments.
• Working knowledge of Support at Home processes, funding and service delivery.
• Working knowledge of NDIS processes, funding and service delivery.
• Understanding of person-centred practice and client choice

Responsibilities

• Develop and maintain positive, respectful and professional relationships with clients and their families, representatives, carers, advocates and other service providers
• Get to know each client's individual needs, preferences, goals, strengths and circumstances
• Ensure clients are involved in decisions about their supports and services
• Promote choice, dignity, independence, reablement and quality of life
• Monitor client wellbeing and identify changes in needs, risks or circumstances
• Respond promptly and appropriately to client concerns and requests
• Advocate for clients where appropriate and support them to access services and resources
• Liaise with FCN Coordinator to arrange client services in accordance with individual support plans and funding arrangements
• Coordinate allied health professionals, contractors and other service providers to provide client support outlined in support plan
• Ensure services are scheduled appropriately and respond to changes in client circumstances
