Qualifications

• Experience in Support at Home or related sector.
• Knowledge of NDIS and/or aged care.
• Excellent communication and time management.
• Proficient computer skills with rostering/CRM systems.
• Eligible to work in Australia without restrictions.

Responsibilities

• Review and provide specialist input on client care plans, ensuring alignment with individual needs, goals, and evidence-based best practice standards.
• Advise care teams on complex care situations, including changes in client physical, cognitive, or mental health status.
• Identify and advise on solutions to complex scheduling challenges, including conflicts, gaps in coverage, or high-risk client scenarios.
• Coordinate and manage the scheduling of client visits and support worker assignments, taking into account availability, skills, geographic location, and client preferences.
• Develop, allocate and manage financial resources and budget utilisation and monitor surplus/deficit strategies.
• Collaborate with Participants, their families, and healthcare professionals to establish and update care goals.
