Job summary

VMCH is seeking a CHSP Intake Coordinator to support the Care at Home Growth function. You will manage regional waitlists, engage with prospective clients to assess needs and readiness, and coordinate information for commencement of services.

Qualifications

• Experience in intake, client coordination or customer service within aged care or a regulated service.
• Ability to manage high-volume waitlists and follow up with accuracy and confidentiality.
• Strong verbal and written communication, with ability to explain eligibility, options and steps.

Responsibilities

• Manage regional CHSP waitlists and opportunity lists.
• Contact prospective clients to confirm needs, eligibility and readiness.
• Facilitate service offers and coordinate information for commencement.
• Maintain accurate client, waitlist and commencement records.
• Coordinate handovers with Service Delivery and rostering teams.
