Job summary

Lifestyle Solutions is hiring a permanent Case Worker based in Broome to support children and young people in Out-of-Home Care through therapeutic case management, frontline coaching and collaborative care planning. You will help create safe, connected and thriving outcomes for young people and their networks.

The role focuses on trauma-informed practice, documentation, and working with families and external agencies to deliver culturally respectful care.

Qualifications

• Diploma in Community Services, Youth Work, or Social Work (or currently pursuing)
• Proven case management and stakeholder engagement skills
• Knowledge of trauma-informed and therapeutic care practices
• Strong documentation, organisational and compliance capabilities
• Current driver's licence and probity clearances
• Empathy, resilience, and a collaborative, solutions-focused approach

Responsibilities

• Lead and review case plans to achieve therapeutic, cultural, and individual goals
• Coach and support frontline staff to embed therapeutic strategies in daily care
• Engage with families, carers, and external agencies to ensure aligned and positive outcomes
• Maintain accurate documentation and ensure compliance with relevant standards and legislation
• Promote safe, nurturing, and culturally respectful environments for children and young people
• Support the delivery of trauma-informed, evidence-based care practices across services
