Job summary

APTOS on the Gold Coast is seeking a reliable Case Manager / Service Coordinator to join our on-site team. You will manage individual cases and coordinate daily services, acting as the primary contact for clients, families and support workers to ensure care remains consistent and high quality.

You will develop care plans, review risk assessments, and liaise with health professionals and providers. This role focuses on person-centred care and safeguarding, with roster coordination and staff

Qualifications

• Experience in case management, service coordination or care coordination within disability, aged care or community services.
• A relevant qualification in Community Services, Disability, Aged Care, Social Work, Nursing or a related field, or equivalent relevant experience.

Responsibilities

• Build positive relationships with clients, families and representatives.
• Coordinate client onboarding, assess service needs and organise appropriate supports.
• Develop, maintain and review care plans and risk assessments in collaboration with clients and professionals.
• Monitor client wellbeing, satisfaction and changing support needs.
• Coordinate services in line with client preferences, goals, service agreements and funding.
• Liaise with support coordinators, health professionals, plan managers and other providers.
• Prepare and maintain rosters, arrange shift coverage and communicate changes promptly.
• Match support workers to clients' needs and skills.
• Ensure staff have access to current care plans and risk assessments.
• Provide day-to-day guidance to support workers and elevate concerns appropriately.
