Job summary

The Salvation Army Australia is seeking a compassionate Case Manager to join the Homelessness Services team at The Open Door and Flagstaff residential facilities in Flagstaff, VIC. You will provide person-centred, strengths-based support to residents experiencing homelessness and develop tailored case plans.

Reporting to the Team Leader, this fixed-term full-time role runs through October 2027. You will coordinate services across housing, health, legal, and community sectors to help clients

Qualifications

• Degree qualification in Social Work, Psychology, Community Services, or a related discipline.
• Demonstrated experience in case management within homelessness, housing, or community services.
• Strong understanding of homelessness, its causes, and evidence-based intervention approaches.
• Experience building effective partnerships across health, housing, legal, and community sectors.
• Excellent communication, relationship-building, and advocacy skills.
• Victorian Driver's Licence
• Employee Working with Children Check

Responsibilities

• Deliver high-quality case management and support planning.
• Empower clients to identify and achieve their housing and wellbeing goals.
• Advocate for access to housing, health, legal, employment, education, and community supports.
• Assist clients to secure medium and long-term housing options.
• Work collaboratively with internal teams and external stakeholders.
• Facilitate case conferences and coordinate integrated support responses.
• Contribute to continuous improvement initiatives and service development.
• Support a positive, collaborative, and client-focused team culture
