Job summary

The Salvation Army is seeking a values-driven Case Manager to support older people at risk of homelessness, helping them access aged care, housing and essential services. You will work within the Care Finders program across Launceston and nearby areas, providing rapid, person-centered support.

This part-time role is based in New Town, Tasmania, offering flexible arrangements and opportunity to contribute to meaningful social outcomes.

Qualifications

• Tertiary qualifications in Social Work, AOD Counselling, Welfare or Psychology, or substantial relevant experience.
• Extensive knowledge of Tasmania's housing and homelessness sector.
• Experience in client-centred case management in housing/homelessness services.

Responsibilities

• Provide high-quality, person-centred support to older people at risk of homelessness.
• Help clients connect with My Aged Care and other supports to improve wellbeing.
• Assist clients to find safe, sustainable housing and access necessary care.
• Collaborate with the Care Finders team and share updates and reports.
• Keep clear records and provide regular reports to the team.

About the role

We are seeking a values-driven Case Manager to provide rapid, person-centred support to older people who are homeless or at risk of homelessness, helping them access aged care, housing, and essential community services. This role forms part of the Care Finders program, supporting vulnerable clients across Launceston to navigate complex systems and improve their wellbeing. This is a part‑time role, located in New Town, Tasmania.

How you will make an impact

Provide high-quality, person-centred support to older people and those at risk of homelessness who need help to access aged care and other services. Help clients connect with My Aged Care and other community supports to improve their wellbeing and independence. Support clients to find safe, sustainable housing and access the care they need to stay well and housed. Work closely with the wider Care Finders team, offering advice and support with more complex client needs. Keep clear records and share regular updates and reports with the Care Finders Team.

What you will bring

• Tertiary qualifications in Social Work, AOD Counselling, Welfare, Psychology, or a related discipline, or substantial relevant experience.
• Extensive knowledge of and experience in the housing and homelessness sector in Tasmania.
• Demonstrated experience in client-centred case management within housing, homelessness, or related community services settings.
• An understanding of relevant Tasmanian and national legislation, policies, and frameworks relating to homelessness, housing, and social support services.
• A current Driver's Licence.
• A working knowledge of client management systems and Microsoft 365 in a professional setting.
• A current Working with Vulnerable People Registration (Tasmania).
