Job summary

About the role

Care finder provides person‑centred support to older people with complex barriers to engagement with the aged care system and who don’t have anyone, such as a friend or family‑member, who can engage on their behalf, helping them access aged care assessments and services, as well as a range of community supports including homelessness services, mental health supports, medical services and alcohol and other drug services. This role forms part of the Care Finder program (see Department of Health, Disabilities and Ageing; My Aged Care), supporting vulnerable older clients across Southern Tasmania to navigate complex systems and improve their wellbeing. This is a full‑time role, located in New Town, Tasmania.

How you will make an impact

Provide high‑quality, person‑centred support to older people including those at risk of homelessness who need help to access aged care and other services. Help clients connect with My Aged Care and other community supports to improve their wellbeing and independence. Support clients to access the care they need to stay well and housed. Work closely with the wider Care Finders team, offering advice and support with more complex client needs.

Key responsibilities & duties

Keep clear records and share regular updates and reports with the Care Finders Team.

What you will bring

• Tertiary qualifications in community services or a related discipline, or substantial relevant experience.
• Working knowledge of the housing and homelessness sector in Tasmania would be beneficial.
• Demonstrated experience in client‑centred case management within community services settings.
• A broad understanding of relevant Tasmanian and national policies, and frameworks relating to aged care, homelessness, housing, and social support services.
• A current Driver’s Licence.
• A working knowledge of client management systems and Microsoft 365 in a professional setting.
• A current Working with Vulnerable People Registration (Tasmania).
• Strong interpersonal skills with the ability to connect with youth from diverse backgrounds.
• Willingness to undertake induction training including First Aid.
