Resolute Support is seeking a dedicated Case Manager to coordinate participant supports in our Community Based Program. You will build strong relationships with participants, their families, and Support Workers to ensure care plans meet needs and funding is optimised.
With a focus on high-quality service delivery, you will maintain accurate records in line with NDIS requirements, participate in team meetings and contribute to continuous improvement across the service.

As part of a dedicated team, you will play a key role in delivering high-quality, person-centered supports.
This position is responsible for the case management and coordination of our participants supports within our Community Based Program, ensuring quality service delivery outcomes met each individual engaged in our services. The role is specifically designed to achieve a high standard of excellence in customer service and consumer satisfaction, through effective communication and coordination of Support Workers.
The role requires strong relationship building with Support Workers and participants through active engagement, and visitations. Organisational and interpersonal skills to engage with our valued participants, their families, carers and representatives is a key priority for this position.
Build and maintain strong professional relationships with participants and their family members, Support Workers, carers and/or advocates, external stakeholders, to understand their preferences and needs, and communicate this with the greater Resolute Support team.
Proactively engage with participants and/or their family members, carers and/or advocates to ensure maximum funding allocations are utilised and assist them to plan their supports (personal care, cleaning, transport, community engagement, etc).
Communicate effectively through regular check-in's and visitations, to ensure participant needs, goals and outcomes are being achieved whilst providing direction and/or support to the Support Worker teams.
Maintain clear and accurate records and documentation of the work you do, in accordance with NDIS and organisational requirements.
Actively participate in team meetings and contribute to the continuous improvement activities at Resolute Support.
2 years experience in a Case Management, Care Co-ordinator role (disability, aged care, youth, mental health).
Minimum Cert III in Community Services, Individual Support (Disability/Aged Care), or relevant qualification.
Have worked in customer service, admin, or coordination roles previously and have the ability to juggle multiple priorities whilst staying organised.
Can have thoughtful, sensitive conversations and resolve problems quickly, with customers interests in mind.
Are comfortable using systems and learning new technology and always working to improve systems and the way you approach things.
Are curious, kind and not afraid to take initiative and/or guidance.

Full-Time Employment(Monday - Friday)
4 Weeks Annual Leave per year.
Optional rostered day off per month!
