Responsibilities

• Supporting and guiding Support Workers.
• Assisting with day-to-day service coordination.
• Communicating effectively with participants, families and staff.
• Helping ensure supports are delivered according to participant needs and preferences.
• Supporting staff with day-to-day questions and concerns.
• Maintaining accurate records, documentation and shift information.
• Promoting a positive, respectful and inclusive environment.
• Following Embrace Community Care policies, procedures and NDIS requirements.
• Identifying and escalating concerns, incidents and issues to management when required.
• Supporting effective communication between staff, participants and management.
• Helping maintain a safe and supportive environment for participants and staff.
