Qualifications

• Experience in case management or care coordination required.
• Strong understanding of person-centred, consumer-directed care.
• Current First Aid and CPR certificates; NDIS Worker Screening Check.

Responsibilities

• Complete non-clinical care planning that captures each participant's needs, goals, and preferences.
• Develop and review care plans with participants and representatives per guidelines.
• Coordinate care teams and external providers to ensure safe, on-schedule services.
• Monitor changing needs and escalate clinical concerns as needed to the Clinical Team Leader.
• Maintain accurate records and track participant budgets in the system.
• Build relationships with GPs, allied health, hospitals and community providers across Gippsland.
• Assist participants to understand rights, navigate systems and provide feedback.
• Contribute to continuous improvement and uphold quality, WHS, and compliance standards.
