MCCGC/CURA Community Services on the Gold Coast is seeking a Full-Time Care Partner to deliver person-centred home care services under SAH and CHSP. You will conduct assessments, care planning, and coordination to support older Australians and their families while respecting diverse cultural needs.

The role emphasizes quality, compliance, and collaboration with health professionals, service providers, and families, with on-call duties as required.

Competenze

• Minimum Certificate IV in Aged Care, Disability, Community Services or related field.
• Minimum 2 years' experience in case management or care coordination.
• Experience with Home Care funding programs (SAH, CHSP, NDIS) and care planning.

Mansioni

• Build strong participant relationships through assessments, reviews, and care planning.
• Coordinate and monitor services to ensure cultural responsiveness and funding compliance.
• Promote participant choice, rights, and informed decision-making.
• Maintain documentation, care plans, case notes, and quality records (AlayaCare).
• Identify risks, escalate clinical concerns, and participate in incident management.
