Job summary

Home Care Assistance Central Coast is seeking a compassionate Care Partner to manage a portfolio of Home Care Package clients across the Central Coast region. You will coordinate services, monitor budgets and build strong relationships with clients and families to promote independence and quality of life at home.

Responsibilities include onboarding, care planning and regular reviews, while collaborating with GPs, nurses and community services to deliver exceptional person-centred care.

Qualifications

• Certificate IV in Community Services or related field with 1–2 years’ Care Partner experience
• EN and RN are welcome and encouraged to apply
• Current national police check (or willingness to obtain)
• Working rights in Australia

Responsibilities

• Onboard clients, conduct assessments and care planning
• Complete regular care plan reviews and monitor changing needs
• Coordinate services with clients and families
• Identify risks and escalate clinical concerns
• Collaborate with allied health providers, GPs, hospitals and community services
• Monitor client budgets and support informed decisions
• Maintain accurate documentation and ensure compliance with aged care requirements
• Support positive client outcomes through proactive case management
