MiCare, a not-for-profit organisation delivering aged care, home care and migrant support across Victoria and Queensland, invites a Care partner to join our values-led team. You will help plan and coordinate care for older clients, ensuring culturally appropriate practices and safety while supporting families.

The role involves initial and ongoing care planning, service coordination, and working with clients and clinicians to connect them to health and community services.

Qualifications

• Tertiary qualification in Social Work, Allied Health or Nursing.
• APHRA registration would be highly regarded.

Responsibilities

• Support care planning and coordination for older clients.
• Ensure culturally appropriate practices and safety in care.
• Assist clients to access required services and manage budgets.
