Job summary

Comlink Australia is seeking a dedicated Care Partner in Melbourne to support older Australians to live independently through coordinated care plans. You will assess needs, build relationships, and navigate funding to achieve participant goals.

The role emphasizes empathy, rigorous attention to detail, and effective budgeting within a collaborative team, with ongoing development opportunities and flexible work arrangements where possible.

Qualifications

• Experience in aged care case management or care coordination.
• Excellent communication and relationship-building skills.

About the role

As a Care Partner, you'll work alongside older Australians to understand what's important to them and coordinate the supports that help them live independently and achieve their goals.

You’ll build trusted relationships, complete assessments and reviews, develop care plans, manage individual budgets and make balanced decisions that consider participant choice, risk and available funding.

You’ll be someone who combines empathy with sound judgement.
