Job summary

SAI Home & Community Care in Victoria is seeking a Registered Nurse with hands-on Support at Home experience to join our team as a Care Partner. You will translate client goals into practical, compliant care plans and build lasting relationships with participants and families.

You’ll work under supportive leaders, using modern digital tools to reduce admin and focus on delivering high-quality, person-centred care across a defined caseload.

• Identify risks or wellbeing concerns early and act decisively
• Coordinate supports and referrals to ensure seamless care
• Manage incidents and feedback with empathy and professionalism
• Maintain accurate documentation that is audit-ready
• Partner with care workers and internal teams to deliver consistent outcomes
• Use digital tools to stay organised and maximise time

The Opportunity

We’re looking for a Registered Nurse with hands-on Support at Home experience to join our team as a Care Partner.

This is an opportunity to combine your clinical expertise and knowledge of the Support at Home program with a genuine passion for building relationships and helping clients achieve the outcomes that matter most to them.

You’ll build genuine face-to-face relationships, understand what matters most to each client, and translate goals into practical support that improves day-to-day life.

• Maintain accurate, high-quality documentation that is current and audit-ready
• Partner closely with care workers, clinicians and internal teams to deliver consistent outcomes
• Use systems and digital tools effectively to stay organised and maximise participant-facing time

You’ll Also Bring

• Experience in aged care, community services, health or a related sector
• A relevant nursing qualification and current AHPRA registration
• Understanding of Home Care Packages and/or the Support at Home program (desirable)
• Confidence managing a caseload and taking ownership of outcomes
• Strong communication and relationship-building skills
• Ability to balance participant advocacy with compliance and funding requirements
• Comfort using digital systems, CRM platforms and care management tools
• Adaptability, resilience and openness to new ways of working
• Clinical, allied health or community services qualifications (desirable)
