We’re looking for a passionate, adaptable and commercially minded Care Partner to join our team and support aged care participants under the Support At Home program.
As a Care Partner with Harbour At Home, you’ll move beyond coordination - building trusted, face‑to‑face relationships with participants and their families, acting as their guide, advocate and strategic partner throughout their care journey.
You’ll focus on outcomes: helping people set meaningful goals, anticipate changing needs, proactively adjust care plans, and make informed choices about how to use their funding in ways that genuinely enhance their daily lives.
You’ll be supported by experienced colleagues and a dedicated manager, clear processes and smart digital tools (including AI‑enabled supports) that free you up to focus on what you do best - listening and connecting, solving problems and delivering high‑quality, goal‑driven care.
This is a role for someone who enjoys learning, is comfortable with change, and wants to help shape how great care is delivered in an evolving sector.
Location: This position will focus on supporting clients in Woolgoolga and surrounds. We're looking for candidates who are based North of Coffs and comfortable to travel and support our team in Grafton.
