Job summary

Harbour At Home in Woolgoolga is seeking a passionate Care Partner to support aged care participants under the Support At Home program. You’ll build trusted relationships, guide and advocate for participants, and work with families to ensure care plans optimise outcomes while managing budgets.

You’ll be supported by experienced colleagues, with clear processes and digital tools, and you'll focus on meaningful goals, proactive planning, and high-quality, person-centred care.

Qualifications

  • Candidates should demonstrate empathy, resilience and strong people skills.
  • Experience with older adults and home care is preferred.
  • Familiar with budget discussions and funding considerations.

Responsibilities

About The Role

We’re looking for a passionate, adaptable and commercially minded Care Partner to join our team and support aged care participants under the Support At Home program.

As a Care Partner with Harbour At Home, you’ll move beyond coordination - building trusted, face‑to‑face relationships with participants and their families, acting as their guide, advocate and strategic partner throughout their care journey.

You’ll focus on outcomes: helping people set meaningful goals, anticipate changing needs, proactively adjust care plans, and make informed choices about how to use their funding in ways that genuinely enhance their daily lives.

You’ll be supported by experienced colleagues and a dedicated manager, clear processes and smart digital tools (including AI‑enabled supports) that free you up to focus on what you do best - listening and connecting, solving problems and delivering high‑quality, goal‑driven care.

This is a role for someone who enjoys learning, is comfortable with change, and wants to help shape how great care is delivered in an evolving sector.

Key responsibilities include:
  • Translate participant goals into clear, practical care plans that are personalised, compliant, and easy to understand.
  • Keep care plans current through regular reviews, proactive check‑ins and timely updates as participant needs change.
  • Actively plan and track participant budgets to support goal achievement, sustainability and value for money.
  • Guide participants through informed decision‑making around services, priorities, trade‑offs and co‑contributions.
  • Identify emerging risks, changes in wellbeing or service gaps early and take timely action.
  • Coordinate services and referrals to ensure care is seamless, reliable and responsive
  • Maintain accurate, high‑quality documentation and records that are always current and audit‑ready.
  • Manage incidents, feedback and complaints with professionalism, empathy and timely follow‑up.
  • Work closely with care workers, clinicians and internal teams to deliver consistent, high‑quality care.
  • Use systems and digital tools effectively to stay organised, meet timeframes and maximise client‑facing time.

Location: This position will focus on supporting clients in Woolgoolga and surrounds. We're looking for candidates who are based North of Coffs and comfortable to travel and support our team in Grafton.

About You

You care deeply about people - and you also enjoy thinking, planning and problem‑solving. You bring empathy, confidence and resilience, and you take pride in helping older people live safely, independently and with dignity at home.

You enjoy partnering with participants rather than “doing things for” them. You’re comfortable having meaningful conversations, including budgets and services, and you approach change as an opportunity to learn and do things better.

Most importantly, you bring a caring heart and a commercial mind - balancing kindness with accountability, and warmth with sound judgement.

You’ll also bring:

  • An understanding of Home Care Packages and/or the Support at Home Program (desirable)
  • Confidence managing a caseload and taking ownership of client outcomes(or desire to learn)
  • Strong communication and relationship‑building skills
  • The ability to balance client advocacy, funding requirements and compliance
  • Comfort using digital systems, CRM platforms and care management tools
  • Resilience, adaptability and openness to new ways of working
  • A clinical, allied health or community services qualification (desirable)
